How Long Does Credentialing Take for Therapy Practices?

One of the most common questions we hear from therapy practice owners is: "How long will credentialing take?" The honest answer is that it depends on several factors, but understanding the typical timeline helps you plan ahead and avoid revenue gaps.

Typical Credentialing Timelines

For most commercial insurance payers, the credentialing process takes between 60 and 120 days from the time a complete application is submitted. However, some payers can take significantly longer, especially during high-volume periods or when additional documentation is requested.

Here's what you can generally expect:

Commercial payers (Aetna, Cigna, BCBS, UHC): 60-120 days on average. Some payers have streamlined their processes, while others are notoriously slow. Blue Cross Blue Shield plans, for example, vary significantly by state.

Medicare: 45-65 days for initial enrollment through PECOS. Revalidation requests are typically processed faster, but delays are common if your application has errors or missing information.

Medicaid: 30-90 days, but this varies enormously by state. Some states have efficient online portals, while others still rely on paper applications and manual processing.

Why Delays Happen

The most common reasons credentialing takes longer than expected:

Incomplete applications are the number one cause of delays. Missing signatures, outdated documents, or incorrect information will result in your application being returned or placed on hold.

CAQH profile issues affect many providers. If your CAQH ProView profile isn't complete and attested, payers will not process your application. Many practices don't realize that CAQH attestation expires every 120 days and must be renewed — our CAQH profile setup guide walks through the whole process.

State license verification can add time if your state licensing board is slow to respond to verification requests. Some states still process these manually.

How to Speed Up the Process

Start the credentialing process as early as possible. Ideally, you should begin 90-120 days before you need a provider to start seeing patients under a specific payer.

Ensure your CAQH profile is complete, accurate, and attested before submitting any payer applications. Most commercial payers pull directly from CAQH, so an incomplete profile will stall every application.

Submit clean, complete applications the first time. Double-check every field, ensure all signatures are present, and include all required supporting documents.

Track every application status systematically. Following up with payers at regular intervals (every 2-3 weeks) can help move stalled applications forward.

Keep copies of everything you submit. If a payer claims they never received your application, having documentation of when and how you submitted it is invaluable.

Planning for New Providers

If you're hiring a new therapist, start the credentialing process before their start date. Have them complete their CAQH profile and gather all required documents during the hiring process, so you can submit applications as soon as possible.

Timelines also differ by discipline, because the credentials a payer verifies differ. See what the process looks like for occupational therapy, physical therapy, speech therapy, ABA therapy and behavioral health practices.

For practices expanding into new states, remember that you'll need state licensure before you can begin the payer credentialing process. Factor in state license processing times when planning your timeline.

The Bottom Line

Credentialing is not something you can rush, but you can make it significantly more efficient by being organized, thorough, and proactive. The practices that credential fastest are the ones that have their documentation in order and follow up consistently. Most of the avoidable delay comes down to a short list of recurring errors — see the five credentialing mistakes that cost practices revenue.

At Credentialing Now, we help therapy practices manage the entire credentialing timeline from initial application through final approval, so you know exactly where every provider stands at every stage. See pricing or request a demo to walk through it with your own provider list.
